Application for Reinstatement ￫ Obtain confirmation from the Academic Advisor  ￫ Obtain confirmation from the Office of International Affairs ￫ Submit to the Graduate School Administration Office

2. Withdrawal

Application for Withdrawal ￫ Obtain confirmation from the Academic Advisor ￫ Obtain confirmation from the Library ￫ Obtain confirmation from the Office of International Affairs ￫ Submit to the Graduate School Administration Office

3. Leave of Absence

Application for Leave of Absence ￫ Obtain confirmation from the Academic Advisor ￫ Obtain confirmation from the Library ￫ Obtain confirmation from the Office of International Affairs ￫ Submit to the Graduate School Administration Office

4. Re-Leave of Absence
